Copley Health Center is a licensed nursing facility providing skilled, intermediate and assisted living.
Established in 1989, our one-story facility is located in a quiet residential neighborhood. We offer over-sized private and semi-private rooms, all of which are conveniently equipped with individual handicapped accessible bathrooms. Also available are dining facilities, a chapel (used by all denominations) and resident lounge areas throughout the facility.

Physician directed medical services
Family Involvement- Routine conferences are held to assure that families are kept informed on the services being provided.
Weekly interdisciplinary team conference to review each residents individual treatment
Financial Option - We are certified for Medicare, Medicaid, self-pay, and most private insurance plans. Our Financial Department is available to answer any questions you may have.
Nutritional management by clinical dietician.

Our Clinical Assessment Nurse will ensure a smooth transition from the community or hospital into the nursing home, by doing the following for your loved one:
Assuring medical information is reviewed for appropriate placement and continuity of care.
Coordinating with hospitals and other health care agencies for a smooth transfer to our facility
Making an appointment for you to complete admission documents
Assuring financial questions are answered.
Providing discharge planning services and including outpatient rehabilitation
Each resident's personal physician is invited and encouraged to continue following the care of the resident while at Copley Health Center. However, a team of highly trained physicians is available to follow the care of the resident if necessary.
